• Provide individualized coaching in daily living skills
• Provide education and support in the use of community resources
• Maintain accurate and current records in ID notes and communication logs regarding information relevant to Individual Service Plans
• Support and assist persons served in following their daily activity schedule including household meal preparation, maintaining sanitary and orderly conditions in their home, other household tasks, budgeting, ledgers, and transportation as necessary
• Prompt and assist persons served in medication management
• Teach adaptive living skills and monitor the extent to which they are followed
• Participate in in-service training and staff meetings to facilitate professional development
• Assist participants in all aspects of personal care, including medical/dental appointments
• Communicate concerns related to program participant needs to supervisor in a timely manner
• Assists individuals in attending social activities to further social skills

Qualifications

• Bachelor’s degree (or equivalent experience) in Psychology, Education, Social Services, or a related field.
• Two (2) years’ experience working with individuals with developmental or other disabilities preferred, including at least one (1) year of administrative and/or supervisory responsibility.
• Proven leadership and motivational skills; able to earn the respect and trust of team members and program participants.
• Excellent written and verbal communication skills; strong attention to detail.
• Proficiency in Microsoft Office (Word, Excel, Teams, etc.).
• Compassion, patience, and respect for individuals with disabilities.
• Valid CA Driver’s License with acceptable record and reliable transportation.
• Must be available in the event of a declared disaster, per agency plan.
• CPR/First Aid certification (training provided).

Physical Requirements

• Ability to stand or sit for long periods; climb stairs; stoop, bend, and lift up to 50 lbs using proper body mechanics.
• Must be alert to environmental cues and able to communicate effectively.
• Ability to implement Crisis Management, Safety, and Pro-Act techniques (training provided).

Immediate Supervisor: Program Manager/Director of Independent and Supported Living Services

Supervision Exercised: None

SUMMARY:

Under the supervision of the Program Director and Manager, this position will provide awake overnight support services to individuals living in independent and supported living settings, promoting safety, independence and dignity for persons served.

ESSENTIAL FUNCTIONS:

• Record and appropriately utilize necessary data, progress notes, staff logs, and program required documentation
• Supervise and assist persons served in following their overnight and daily morning schedule including household meal preparation, maintaining sanitary and orderly conditions in their home, other household tasks and transportation as necessary
• Prompt and assist persons served in medication management as needed
• Assists participants in all aspects of personal care
• Assist persons served in weight bearing and non-weight bearing transfers
• Use crisis intervention techniques in emergency situations when persons served are in danger of injuring themselves or others
• Participates in in-service training and staff meetings to facilitate professional development

• Complies with all Momentum Work Inc. Policies and OSHA policies and procedures

• Other related duties, responsibilities and special projects on an as needed basis

PHYSICAL REQUIREMENTS

• May need to be available during any disasters, per disaster plan
• Must have reliable phone
• Must have personal car and valid insurance
