SUMMARY:
Under the supervision of the Director of Employment Services, this position is responsible for carrying out objectives of evaluation and training and maintaining a high standard of programming. The Job Coach is responsible for coaching individual placements, ensuring quality, program participant driven placements and customer satisfaction.
ESSENTIAL FUNCTIONS
- Monitors proper implementation and documentation of the Individual Service Plans for each program participant
- Coordinates the overall implementation of the program as it relates to the goals of each program participant in his/her ISP in conjunction with Employment Services Management
- Ensures that program components meet the assessed needs of the program participants in their ISP in conjunction with Employment Services Management
- Assists in compliance with Agency policies and CARF regulations
- Responds promptly to emergency situations
- Uses crisis intervention techniques in emergency situations when program participants are in danger of injury to themselves or others
- Ensures that program participant notes are completed in a timely manner
- Acts as a liaison between the employer, persons served and Momentum WORK, Inc.
- Attends scheduled meetings and in-service trainings as required
- Handles approved petty cash disbursement as it relates to program implementation
- Complies with all Momentum WORK Inc. policies and procedures
- Completes all required paperwork in a timely manner
- Other related duties, responsibilities, and special projects on an as needed basis
PHYSICAL REQUIREMENTS:
- Ability to stand, sit for long periods of time
- Ability to go up and down stairs
- Ability to walk for long periods of time
- Ability to push or pull wheelchairs when necessary
- Ability to speak, read, hear, and understand technical written materials
- The employee is frequently required to climb, stoop, twist, or bend and lift up to fifty (50) pounds, using proper body mechanics to minimize injuries to self and people served
- Full awareness of environmental stimuli
- Ability to implement Crisis Management, Safety & Pro Act techniques (training included)

QUALIFICATIONS :
- High school diploma or equivalent
- At least one (1) year of experience working with persons with developmental and/or other disabilities in individual and/or group settings preferred
- Demonstrated ability to maintain the highest standards of confidentiality regarding persons served and other related information
- Excellent verbal and written communication skills
- Ability to complete tasks with numerous interruptions
- Compassion and respect for persons with disabilities
- Ability to demonstrate patience and understanding because people served may have intellectual, emotional, or behavioral challenges, in addition to physical limitations
- May need to be available during any disasters, per disaster plan
- Must have reliable phone
- Valid CA Driver’s License: must have an acceptable driving record as determined by criteria established by the Agency’s insurance carrier and by Agency policy
- Valid certification in CPR/First Aid (Training included)
