TITLE: Job Coach, Harbor Crew

Job Type: Part Time

Immediate Supervisor: Manager, Harbor Crew

Supervision Exercised: Harbor Group Workers (Person Served)

SUMMARY:

Under the supervision of the Harbor Manager, the Harbor Crew Coach is responsible for providing vocational training of individuals served by Momentum WORK, Inc., as well as ensuring that contract specifications are met and exceeded.  The Harbor Crew Coach is also responsible for the implementation of Individual Service Plans and ensuring the crew receives quality-training services that promote independence, choice, and quality of life.

ESSENTIAL FUNCTIONS

  • Supervises and instructs the group of workers in all aspects of the contract
  • Ensures that all workers have been properly trained and understand their job duties
  • Provides initial orientation and ongoing safety training
  • Ensures that equipment and supplies are accessible and in good/safe condition
  • Responsible for supervision and/or assigning supervision of crew members at all times (breaks included)
  • Liaison with Harbor Organization personnel
  • Attends in-service training, staff meetings, crew member annual meetings and planning sessions
  • In collaboration with Case Management, ensure that behavior programs, schedules, goals and other reports are completed and implemented
  • Conducts individualized training for skill and work habit development
  • Supervises and monitors the successful completion of contract specifications
  • Works in collaboration with the Management Team in ensuring coverage for the work site
  • Complies with all Momentum WORK, Inc. and CARF policies and procedures
  • Completes all required paperwork
  • Other related duties as assigned

PHYSICAL REQUIREMENTS:

  • Ability to stand, sit for long periods of time
  • Ability to go up and down stairs
  • Ability to walk for long periods of time
  • Ability to push or pull wheelchairs when necessary
  • Ability to speak, read, hear, and understand technical written materials
  • The employee is frequently required to climb, stoop, twist, or bend and lift up to fifty (50) pounds, using proper body mechanics to minimize injuries to self and people served
  • Full awareness of environmental stimuli
  • Ability to implement Crisis Management, Safety & Pro Act techniques (training included)

QUALIFICATIONS :

  • High school diploma or equivalent
  • At least one (1) year of experience working with persons with developmental and/or other disabilities in individual and/or group settings preferred
  • Demonstrated ability to maintain the highest standards of confidentiality regarding persons served and other related information
  • Excellent verbal and written communication skills
  • Ability to complete tasks with numerous interruptions
  • Compassion and respect for persons with disabilities
  • Ability to demonstrate patience and understanding because people served may have intellectual, emotional, or behavioral challenges, in addition to physical limitations
  • May need to be available during any disasters, per disaster plan
  • Must have reliable phone
  • Valid certification in CPR/First Aid (Training included)
