TITLE:  Case Manager, Employment Services

Job Type: Part Time, Up to 20 hours a week

Immediate Supervisor:  Director of Case Management

Supervision Exercised: None

SUMMARY:

Under the supervision of the Director of Case Management, this position is responsible for the overall case management of program participants in their area.

ESSENTIAL FUNCTIONS:

  • Provides program tours for participants interested in receiving services, completes intake process, and maintains waitlists as needed
  • Creates and maintains Case Magic files for every participant in all programs, including Individual Support Plan, Medical Records, Medication Lists, emergency contacts, and all relevant personal information
  • Helps each participant create an Individual Support Plan and update it at least semi-annually
  • Ensures that program components meet the assessed needs of the program participants in his/her ISP in conjunction with the Program Director
  • Completes assessments and/or arranges for designated staff to complete assessment
  • Obtains and maintains working knowledge of generic services (MediCal, Medicare, Social Security, SSI, IHSS etc.)
  • Assists in compliance with agency policies and procedures, TCRC, CCL, and CARF regulations
  • Maintains regular contact and fosters positive interaction with external funding agencies and community partners, meeting on a regular basis with agency representatives
  • Understands relevant hardware (iPad, WiFi hotspot, etc.) and software (Case Magic, Zoom, Teams, MS Office, Box, etc.)
  • Responds promptly to emergency situations
  • Uses crisis intervention techniques in emergency situations when program participants are in danger of injury to themselves or others
  • Ensures that all program and agency documentation is completed and turned in a timely manner
  • Conducts intake for program participants
  • During absence, ensure that all responsibilities are delegated accordingly
  • Prepares program semi-annual and annual reports as necessary
  • Participates in the development of program support plans
  • Attends scheduled meetings and in-service trainings as required
  • Knowledge and use of technology
  • Complies with all Momentum WORK, Inc., TCRC and CCL policies and procedures
  • Other related duties on an as needed basis

PHYSICAL REQUIREMENTS:

  • Ability to stand, sit for long periods of time
  • Ability to go up and down stairs
  • Ability to speak, read, hear, and understand technical written material
  • The employee is frequently required to climb, stoop, twist, or bend and lift up to fifty (50) pounds, using proper body mechanics to minimize injuries to self and people served
  • Full awareness of environmental stimuli
  • Ability to implement Crisis Management, Safety & ProAct techniques (training included)

QUALIFICATIONS :

  • Bachelor’s Degree or equivalent with an emphasis in the areas of education, psychology, social services, or related field (work experience may be substituted for education)
  • Two (2) years’ experience working with persons with developmental and/or other disabilities in individual and/or group settings preferred; One (1) year of w year of which must be increasingly responsible for administrative and/or supervisory experience
  • Proven ability to work with and motivate staff members
  • Ability to earn respect and confidence of Momentum WORK, Inc. staff
  • Demonstrated ability to maintain the highest standards of confidentiality regarding persons served and other related information
  • Demonstrated ability to communicate effectively with all levels of staff within the organization
  • Excellent verbal and written communication skills
  • Excellent computer proficiency (esp. Microsoft Office – Word, Excel, Teams etc.)
  • Compassion and respect for persons with disabilities
  • Ability to demonstrate patience and understanding because people served may have intellectual, emotional, or behavioral challenges, in addition to physical limitations.
  • Will need to be available during any disasters, per disaster plan
  • Valid CA Driver’s License: must have an acceptable driving record as determined by criteria established by the Agency’s insurance carrier and by Agency policy
  • Valid certification in CPR/First Aid (Training included)
