TITLE: LEAD STAFF – RESIDENTIAL SERVICES 

Full-Time Position

Immediate Supervisor: Director or Manager, Residential Services

Supervision Exercised: None

SUMMARY:

In accordance with the organizational structure of the Agency and under the direct supervision of the Director or Manager, this position is responsible for the implementation of Individual Service Plans and Person-Centered services that will lead to or continue to promote independence, residential and personal skills and assist in providing a safe and secure home environment.

ESSENTIAL FUNCTIONS:

· Lead by example and coach residential direct care staff

· Assists with schedules and coverage for residential homes

· Attends regular check in meetings with Manager/Director

· Reviews, monitors, and records the administration/distribution and reordering of medications

· Assists in maintaining weight logs, water intake logs, seizure logs, blood pressure logs, daily notes, behavioral documentation, and medical appointment checklists

· Maintains residents’ ledgers and files in compliance with TCRC & CCL

· Assists residents with medical appointments

· Assists with weekly grocery shopping and meal planning activities

· Assists with monthly resident meetings

· Assists with maintaining emergency supplies, monthly inspections, water temperature checks, refrigerator, and freezer temperatures

· Provides education and support in the use of community resources

· Maintains accurate and current records training, supports, hours, integration levels, monies (if the individual is not able to handle their own finances), and Individual Service Plans

· Participates in in-service training and staff meetings to facilitate professional development

· Acts as a liaison between guardians, parents, funding sources, licensing, community members, and neighbors

· Uses crisis intervention techniques in emergency situations when individuals are in danger of injuring themselves of others

· During absence, ensure that all responsibilities are delegated accordingly

· Assists with safety inspections and disaster drills

· Complies with all Momentum WORK Inc., CCL & OSHA policies, procedures, and regulations

· Ensure all common areas in house are organized and clean at the end of shift.

· Other related duties, responsibilities, and special projects on an as needed basis

PHYSICAL REQUIREMENTS

· Ability to stand, sit for long periods of time

· Ability to go up and down stairs

· Ability to walk for long periods of time

· Ability to push or pull wheelchairs when necessary

· Ability to speak, read, hear, and understand technical written materials

· The employee is frequently required to climb, stoop, twist, or bend and lift up to fifty (50) pounds, using proper body mechanics to minimize injuries to self and people served

· Full awareness of environmental stimuli

· Ability to implement Crisis Management, Safety & ProAct techniques (training included)

QUALIFICATIONS :

· High School Diploma or GED required; AA degree preferred

· At least one (1) year of experience working with persons with developmental and/or other disabilities in individual and/or group settings preferred

· State of California DSP II certification

· Demonstrated ability to maintain the highest standards of confidentiality regarding persons served and other related information

· Excellent verbal and written communication skills

· Ability to complete tasks with numerous interruptions

· Compassion and respect for persons with disabilities

· Ability to demonstrate patience and understanding because people served may have intellectual, emotional, or behavioral challenges, in addition to physical limitations

· May need to be available during any disasters, per disaster plan

· Must have reliable phone

· Valid CA Driver’s License: must have an acceptable driving record as determined by criteria established by the Agency’s insurance carrier and by Agency policy

· Valid certification in CPR/First Aid (Training included)
