TITLE: Program Manager, Independent and Supported Living Services
Job Type: Full Time- Exempt
Immediate Supervisor: Director, Independent and Supported Living Services
Supervision Exercised: Direct Support Professional I/II/III, Program Coordinator
SUMMARY:
Under the supervision of the Director of Independent and Supported Living Services, this position is responsible for the coordination of independent and supported living services in the implementation of Individual Service Plans that will lead to or continue to promote independence, residential and personal skills and assist in providing a safe and secure home environment.
ESSENTIAL FUNCTIONS
- Hires, supervises and coaches supported living direct care staff
- Monitors and evaluates staff performance
- Attends management meetings, maintains office days
- Conducts staff meetings, reviews/approves timecards, and mileage sheet
- Prepares incident, medical appt./medication, ISP, biannual and quarterly reports
- Reviews, monitors, and records the administration/distribution, reordering of medication
- Records and inspects biannually staff’s vehicle, driver’s license, and insurance
- Provides individualized coaching in daily living skills
- Provides education and support in the use of community resources
- Maintains accurate and current records on training, supports, hours needed, SL grids, integration levels and Individual Service Plans
- Monitors and completes money ledgers as needed
- Determines the types of supports necessary to maintain current living arrangements
- Provides supported living/transition training in the following: menu planning/meal preparation, socialization, budgeting, conflict resolution, etc.
- Teaches adaptive living skills and monitors the extent to which they are followed
- Participates in in-service training and staff meetings to facilitate professional development
- Assists persons served in all aspects of personal/medical care as needed
- Trains persons served in IL/SL to access public transportation and assists them in accessing benefits such as Social Security, SSI, and Section 8 Housing
- Acts as a liaison between guardians, parents, community, and neighborhood
- Assists in weight bearing and non-weight bearing transfers
- Uses crisis intervention techniques in emergency situations when persons served are in danger of injuring themselves or others
- During absence, ensure that all responsibilities are delegated accordingly
- Works cooperatively with Agency staff, volunteers maximizing experience while meeting the needs of Momentum WORK, Inc. persons served and programs
- Knowledge and use of technology
- Provides direct service support minimum 8 hours weekly and/or coverage as needed
- Assumes other duties and responsibilities as necessary
PHYSICAL REQUIREMENTS:
- Ability to stand, sit for long periods of time
- Ability to go up and down stairs
- Ability to speak, read, hear, and understand technical written material
- The employee is frequently required to climb, stoop, twist, or bend and lift up to fifty (50) pounds, using proper body mechanics to minimize injuries to self and people served
- Full awareness of environmental stimuli
- Ability to implement Crisis Management, Safety & ProAct techniques (training included)

QUALIFICATIONS :
- Bachelor’s Degree or equivalent with an emphasis in the areas of education, psychology, social services, or related field (work experience may be substituted for education)
- Proven ability to work with and motivate staff members
- Ability to earn respect and confidence of Momentum WORK, Inc. staff
- Demonstrated ability to communicate effectively with all levels of staff within the organization
- Excellent verbal and written communication skills
- Excellent computer proficiency (esp. Microsoft Office – Word, Excel, Teams etc.)
- Compassion and respect for persons with disabilities
- Will need to be available during any disasters, per disaster plan
- Valid CA Driver’s License: must have an acceptable driving record as determined by criteria established by the Agency’s insurance carrier and by Agency policy
- Two (2) years’ experience working with persons with developmental and/or other disabilities in individual and/or group settings preferred; One (1) year of w year of which must be increasingly responsible for administrative and/or supervisory experience
- Demonstrated ability to maintain the highest standards of confidentiality regarding persons served and other related information
- Valid certification in CPR/First Aid (Training included)
- Ability to demonstrate patience and understanding because people served may have intellectual, emotional, or behavioral challenges, in addition to physical limitations.
